Below each form name is a detailed description of the form, who is most likely to need the form and a download link to the actual form. If you are having any problems with any of our forms, please contact us for help.

NCAFA Game Forms

Clubs may download the following three forms required at each game. It is our intention to have the scores posted on the site live as they are reported to us by managers/conveynors. Please email these forms in as soon as possible after the game (see your club president for further instructions).

Who needs these documents?

Usually the convenor from each team will need these forms.

NCAFA Constitution

This document contains the NCAFA Constitution, By-Laws and Rules & Regulations. Please save it to your hard drive for future reference. The constitution is updated twice a year.

Who needs the NCAFA Constitution?

All clubs should have a copy for each team level. Coaches, convenors and team managers should also have a copy of the NCAFA constitution.

NCAFA Development Process

This document provides an overview of how the NCAFA is organized. It describes our Vision, Core Values, Mission Statement and presents a graphic overview of the progression players can expect in each level of our Tackle Football program. In addition, the NCAFA "pyramid" provides an excellent view of how our League is organized and the inter-relationships with High School, OVFL, Junior Football and finally, CIS Football programs.

NCAFA Code of Conduct

You may obtain a copy of the NCAFA Code of Conduct from your club or click below to download it and print it. Read it, sign it and bring it with your registration form to your club. If you have any questions regarding this Code of Conduct please see a club official.

Who needs the NCAFA Code of Conduct?

The Code of Conduct is intended to be read and accepted by all league participants, including players, parents/guardians, coaches, administrators, volunteers, and officials.
